Rehabilitation Program for Joint Replacement
Camp Trinity is the name of a special program for patients with hip and knee replacements. Beginning with preoperative education, participants receive information on how to prepare for their patient experience. Preparation may include physician office visits, lab work, x-rays and implementation of safety measures in the home. Postoperative education includes information on self-cares to include diet, exercise, medication management and physician follow up visits.
Camp Trinity utilizes a "team approach" to treatment. The "team" may include an orthopaedic surgeon, hospitalists, physical therapists, occupational therapists, nurses, case managers, dieticians, pharmacists or other healthcare professionals as necessary. Patients who undergo joint replacement surgery and who have participated in Camp Trinity generally feel well prepared for surgery and are happy with their patient experience.
Camp Trinity participants are actively involved in care planning and goal setting during their hospitalization and at the time of discharge. Referrals may be made to other services in and around the community such as home health care, skilled care, or outpatient therapy to assist with rehabilitation and immediate discharge care needs.
The overall goal for the Camp Trinity participant is "best outcome for every patient every time."
