Before You Leave Trinity

Discharge Instructions

• Planning for going home from the hospital begins when admitted. You and your doctor will decide when you are able to leave the hospital.

• If your doctor tells you in the morning that you may leave, we ask that you leave before noon or as soon as arrangements are done.

• Your nurse and other members of the health care team will give you steps to follow at home. If you have any questions or concerns, please ask them before you go home.

• After you are at home, you will get a survey about your stay at the hospital. What you say is important and we hope you will fill out the survey. Your answers are used to help us give better care to our patients.
