Case Management

Each inpatient unit has case managers who are nurses or social workers. They are responsible for coordinating your care, your home instructions, and services you may need at home.

Your case manager will visit you within 24 hours of being admitted.

When going home, some patients will need more services. A case manager will help you in your choice of services for follow-up care.
Spiritual Care
-
A hospital chaplain is on-call 24 hours a day and can be reached by calling the Spiritual Care Department.

The hospital Chapel is on the first floor next to the Critical Care Waiting Area and is open 24 hours a day.

Sacraments may be given when asked.

Home Health Care

Trinity’s Home Health Care is a Medicare certified home care agency that has nurses, nurses’ aides, medical social workers and therapists who visit you in your own home. For more details, ask your doctor, nurse or case manager.

Lifeline is a personal emergency response system for the frail elderly and those with medical concerns. Lifeline offers patients the security of staying in their home and living on their own knowing that with a push of a button, they can get help right away 24 hours a day, seven days a week.
